Place all the ingredients in a 2.5-4 quart crock pot.
Cover and cook on low for 1-2 hours (stirring frequently) until the dip is fully melted and combined.
Once the dip is ready, switch to keep warm on your crock pot and serve with chips.

Notes
Once you are ready to serve, you can add 1/2 cup of Milk if you prefer your dip to be thinner. I also gradually add in milk until the dip is the consistency that you prefer.
